MMR和结核病信息
MMR(腮腺炎、麻疹和风疹)免疫接种
所有学生都必须提交两剂MMR疫苗的证明, an Arkansas Department of Health (ADH) approved immunization exemption (medical, religious or philosophical), 或1957年以前出生的。 For Arkansas residents, NWACC will attempt to retrieve immunization records from a state-wide database.
Acceptable proof of MMR immunization is documentation issued by a licensed medical doctor, an authorized public health representative, an Arkansas education institution (high school or college) or the United States Military. Documentation must reflect the student’s name, at least 1 piece of personal identifiable information outside of the student's name, the vaccine type and the dates that the vaccines were administered.
To protect the privacy of your information and to comply with U.S. Department of Education regulations, we can no longer accept documents by email. You can submit them one of three ways:- 上传文件至: https://api.nwacc.edu/lss/finaid/filedropbox/

- Bring the document to the Enrollment Support Center located on the 2nd floor of the Student Center or the Washington County location located in Springdale

Students with an ADH approved medical, religious or philosophical exemption are exempt from this policy. Students may find more information on the 抗利尿激素的网页 or call 501-661-2169. ADH approved religious and philosophical immunization exemptions require an annual application through ADH. ADH approved medical immunization exemption does not require an annual application.
Exceptions: Students who previously received two doses of measles, one dose of mumps and one does of rubella before January 1, 2010 will be considered compliant and are not required to submit documentation with a 2nd MMR.
结核病筛查
All foreign-born students born in a country where Tuberculosis (TB) is endemic, who are not U.S. Citizens or Permanent Residents, are required to submit TB test results within 30 days of the first semester of which they are enrolled.
Students who have had the required screening performed in the United States or Canada within the six months prior to enrollment will not be required to have it repeated, provided that they present satisfactory documentation.
Acceptable proof of TB screening results is documentation issued by a licensed medical doctor, an authorized public health representative or the United States Military. Documentation must reflect the student’s name, at least 1 piece of personal identifiable information outside of the student's name, the date it was applied, the date the test result was determined and the results. A negative chest x-ray is not a substitute for a skin test.
When a TB skin test (TST) causes 10mm or more of induration or an Interferon-Gama Release Assay (IGR) test is positive, the student will need to have the Arkansas Certification of Tuberculosis Screening for Institutions of Higher Education and addendum completed by a physician and returned to NWACC within 10 business days. Students who test positive for TB are not permitted to visit any of the NWACC Campus locations until the Arkansas Certification of Tuberculosis Screening for Institutions of Higher Education and addendum is completed by a physician and submitted to NWACC reflecting that the student is not contagious and can return on-campus.
TB is not endemic in the countries listed below and students born in these countries are NOT required to submit TB test results.
